Get-Acquainted Information
On our first visit we will ask you for personal
data and health background information. This
information is designed to help us complete
insurance forms or other correspondence
required. The health information provides a
baseline history to aid us in providing better
dental care for you. Please keep us informed
regularly of any changes in name, address,
phone number, or medical condition.
Recare Visits
We are committed to the philosophy
of preventative dentistry. We strongly
recommend all our patients be re-examined
and have their teeth thoroughly cleaned at
appropriately scheduled intervals. The
length of these intervals is determined by
each patient’s personal dental needs.
Appointments / In Case of Emergency
Our office number is 596-6633. Office hours are from 8:30 a.m. to 5:00 p.m. If you have an emergency after hours, please
call the office. If you cannot
keep a scheduled appointment, please give us
at least 24 hours notice so that someone else
may utilize your appointment time.
Fees, Payment, and Insurance
As a courtesy to our patients, our office will
file your insurance claim with your respective
company, but we consider the patient
responsible for the account.
